Every trustee and responsible person on your NFA gun trust must submit ATF Form 5320.23 along with passport-style photos and fingerprint cards. Use this tool to organize your information before filling out eForms.
Each responsible person must submit their own Form 5320.23. If your trust has multiple trustees or responsible persons, each one needs to complete a separate form with their own passport photos and fingerprint cards.

Each responsible person must include these items with their Form 5320.23 submission:
One passport-style photo (2x2 inches)
Must be taken within the last 6 months. White background, front-facing, no glasses.
Two FD-258 fingerprint cards
Must be completed on official FBI FD-258 cards. Rolled and flat impressions required.
Copy of trust document (first and last pages)
Include the cover page, signature page, and schedule of beneficiaries at minimum.
Bring your own FD-258 cards or ask if the location provides them. You need two completed cards per person.
